1. Who we are
HospiceReferrals.com is operated by HospiceReferrals.com ("we", "us"). This policy explains what information we collect on this website, how we use it, and the choices you have.
This website is for hospice providers and other organizations interested in our marketing services. It is not intended for patients or families, and it is not intended to collect any patient or health information. Please do not submit patient information through this website.
2. Information we collect
Information you give us. When you request a Referral Growth Assessment, we collect the details you enter: your name, work email address, phone number, hospice or organization name, primary service area, and anything you choose to write in the message field.
Information collected automatically. When you visit, our servers record technical information such as your IP address, browser type and version (user agent), the page you came from (referrer), the pages you view, and timestamps. If you arrived from an advertisement, we also record campaign parameters in the page address (such as utm_source, utm_campaign and Google's click identifiers gclid, gbraid and wbraid), and we keep them in your browser's session storage and a first-party cookie named hr_attr so that they can be attached to your request if you submit the form.
Information from cookies and similar technologies. We also set strictly necessary first-party cookies that protect the form against forgery (hr.af) and carry your confirmation to the thank-you page (hr.td).
Global Privacy Control. We detect whether your browser sends a Global Privacy Control (GPC) signal and record that state so that we can honor it (see section 6).
3. How we use information
We use the information above to respond to your request and schedule your assessment; to communicate with you about our services; to deliver services if you become a client; to measure the performance of our advertising; to keep the website secure, prevent abuse and diagnose problems; and to comply with legal obligations.
We do not sell personal information. We do not use personal information for automated decisions that have legal or similarly significant effects.
4. Google tag, Google Ads and Google Analytics
This website does not currently load Google advertising or analytics tags. If that changes, this section will be updated before the tags are enabled.

You can delete or block cookies in your browser settings. Blocking the strictly necessary cookies will prevent the assessment form from working.
6. Global Privacy Control and Do Not Track
We honor Global Privacy Control signals. When your browser sends a GPC signal we treat it as a request to opt out of the sale or sharing of your personal information and of targeted advertising, and we do not send any identifying information to advertising platforms. Browsers' older "Do Not Track" setting is not a standardized signal and we do not change our behavior in response to it; GPC is the signal we honor.
7. Google Fonts
Pages on this website load typefaces from Google Fonts. Your browser requests stylesheet files from fonts.googleapis.com and font files from fonts.gstatic.com, and those requests carry your IP address, browser user agent and the referring page to Google. Google states that the Google Fonts API does not set cookies. See https://developers.google.com/fonts/faq/privacy.
8. How long we keep information
Server logs that contain IP addresses are kept for security and troubleshooting and are deleted or anonymized after no more than 90 days unless needed to investigate abuse. Assessment requests are kept for as long as needed to respond to you and, if you become a client, for the duration of our relationship and as required by law; you can ask us to delete your request at any time (section 10).
9. Who we share information with
We share information only with service providers that help us run this website and respond to you, such as web hosting, email delivery, and (if configured) the customer relationship management system that receives form submissions. These providers act on our instructions. We may also disclose information if required by law, to protect our rights, or in connection with a merger, acquisition or sale of our business.
10. Your choices and rights
You can ask us to access, correct or delete the personal information we hold about you, or to stop contacting you, at any time. Reply to any message we send you and we will respond within 45 days.
California residents. The California Consumer Privacy Act applies to businesses that meet certain thresholds. Whether or not we meet them, we honor GPC signals as an opt-out of sale or sharing, we do not sell personal information, and we will honor requests to know, correct and delete personal information made through the contact methods above. We will not discriminate against you for exercising these rights. You may use an authorized agent to make a request.
Residents of other states. If the privacy law of your state gives you rights over your personal information, you can exercise them through the same contact methods.
11. Children
This website is directed to businesses and is not intended for anyone under 16. We do not knowingly collect personal information from children.
12. Security
We use HTTPS for every page and for form submissions, restrict access to submitted requests to people who need it, and store them in protected systems. No method of transmission or storage is completely secure, and we cannot guarantee absolute security. If a breach affecting your information occurs, we will notify you as required by law.
13. Changes to this policy
When we make material changes we will update the effective date above and, where appropriate, note the change on this page.
